Salus Global Corporation (SGC) is committed to keeping your personal information safe and confidential in accordance with applicable federal and provincial/state legislation. Your participation in the MOREOB Program requires that certain types of information be collected at a site designated for the purpose of storing the information. By participating in the program you agree to the terms of the privacy statement that follows.
Web and database security measures will safeguard your personal data. The information collected at the designated site includes personal information such as your name, email address and practice location, including the hospital where you will be participating in the program. It will also include the results of any of the activities in which you have participated in relation to the program. In addition it will include a log of your participation. This log of your participation will contain information about your participation, but not the results of your participation.
Test results will be stored in a secure file together with your personal information and will not be released to any party without your personal written consent. The results will be accessible to you only via a password specifically assigned to you. Your test results will be used to create aggregate test reports. These reports will be available to your home institution as well as to SGC as it relates to program administration and for assessment purposes. These test results will be aggregated in such a manner that your personal information will not be disclosed. A report summarizing your participation history including which activities in which you have participated will be accessible to the core team at your home institution and any other authority responsible for administering the MOREOB Program for your site. Activity reports will include your name. All statistical and aggregate information does not contain personally identifiable or private information and does not contain any individual results.
Access to program information is available only to selected individuals for the purposes outlined above. The information is disclosed only to the extent required by legal requirements, such as laws or valid search warrants.
Of course, you may access all of the information pertaining to your participation in the program.
You should be aware that every effort has been made to ensure the security of your information and data including limiting access and the use of appropriate security measures. SGC will take every precaution to ensure that its database and web applications are secure. Ongoing review and due diligence of the processes, infrastructure and systems design of the MOREOB applications are routinely undertaken. As you are aware, there is always an element of risk in sending data over the internet. The MOREOB security policy may be revised over time as new features are added and/or as web site and internet security and privacy standards evolve. We will post changes prominently so that you will always know what information we gather, how we might use that information and whether we will disclose the information to anyone.
